ELIGIBILITY

 

• Be On-site 15 minutes before your appointment for check-in with your HealthCard or Identification

• ​Please wear a mask when entering the pharmacy and clinic premises.

 

• Sanitize your hands and observe physical distancing.

 

• All appointments are pre-booked

For 1st Dose of Vaccine: 

​

• 18 years and older 

​

For 2nd Dose of Vaccine:

​

• AstraZeneca: The second dose is at least  56 days from the first dose

​

• Moderna/Pfizer: The second dose is at least  21 days from the first dose

​

• We will not be able to give the second shot if it is less than the recommended schedule.
